The Day Of Admission
1. You, or the Power of Attorney for the person planning to be admitted, should come to the Delaware Nursing & Rehabilitation Center to complete the necessary admission paperwork. This should be done before the time of admission so that all the appropriate information may be exchanged. Please bring with you the following cards/papers:
- Medicaid Card
- Medicare Card
- Insurance Cards: Blue Cross, Blue Shield, AARP, etc.
- Recorded Power of Attorney (If you do not have a Power of Attorney we can give you information on appointing one)
2. You should choose and contact a physician who will agree to care for you or your loved one during your stay at the Delaware Nursing & Rehabilitation Center. If you are unsure which physicians are available, please contact our admission department.
3. You may be required to pay a security deposit of one full month, as well as payment in advance for the month of admission, depending upon source of payment.
4. You should plan on bringing clothing and other personal articles that will help make you comfortable and make your room accommodations feel like home.
5. We also recommend that you open up a personal account for your loved one, which can be used for hair care and other personal needs
