A pre-admission assessment will be conducted by a social worker and a registered nurse. The Application for Admission must also be completed. When a bed becomes available, and an applicant is chosen by the Admissions Committee, the Facility Representative will contact the referring agency and/or family to finalize arrangements for admission.
